In addition, access to information is restricted to benefitexpress employees and contractors on a "need to know" basis. These employees include benefitexpress' service center staff, health and benefit plan administrators, and database administrators. All transactions are logged and reviewed by security staff.
Our employees and contractors are informed of their responsibility to protect your information and are governed by a code of conduct that includes this confidentiality. Appropriate disciplinary policies are in place to help enforce employee privacy responsibilities. Complaints about suspected privacy violations may be made directly to benefitexpress via e-mail addressed to firstname.lastname@example.org.
Information Disclosed to Third Parties
When you select or change your benefit coverage through benefitexpress, this information will be transmitted to two other parties. Once you've selected a benefit plan, benefitexpress will send to the employer and plan provider only the information that is necessary for your health benefit coverage to take effect. Typically, this information is limited to your name, age, address, telephone number, Social Security Number, dependents covered, the name of your primary care physician, and the cost of coverage. In addition, benefitexpress occasionally works with sub-contractors on systems' administration. These sub-contractors, by contract, strictly adhere to benefitexpress' policies for security and privacy.
You may choose to use benefitexpress service center staff to help resolve service or claims payment issues. In this instance, you may be required to provide personal health information to benefitexpress to receive this assistance. This is a voluntary action on your part and requesting assistance authorizes us to receive and share this information with your health care provider and/or the claims payer to resolve the issues. Individually identifiable health information will NOT be provided to your employer.
The only time we provide or transmit personal information is at your direction, as necessary for enrollment purposes or as required by law through subpoena, administrative action, or the existence of a clear physical threat. benefitexpress does not resell, trade, lease or rent our user's personally identifiable information to other companies.
Please remember, however, that any information (including personal and medical information) that you reveal in a public forum (such as a bulletin board, mail list, chat room/event) is not subject to this policy and will be seen by third parties not related to benefitexpress and may be used by them to contact you or for unauthorized purposes. Also, if you link to a third party site from benefitexpress, any information you reveal on that site is not subject to this policy. You should consult the privacy policies of each site you visit.
From time to time, you may explicitly elect to have benefitexpress service center staff or an HR representative e-mail you regarding an issue you have raised. These communications may contain specific details of your health or health status. YOU SHOULD BE AWARE THAT ALL E-MAILS YOU RECEIVE AT YOUR PLACE OF EMPLOYMENT MAY RESIDE ON YOUR COMPANY'S E-MAIL SERVER, AND SOME COMPANIES FILTER E-MAIL. CHECK YOUR COMPANY'S POLICY IN THIS REGARD. You can instruct benefitexpress to send e-mail messages to your home e-mail address, if you have one. You should be aware, that any such e-mail may not be encrypted and confidentiality cannot be assured.
Accessing and Correcting Your Personal Data
benefitexpress is committed to securely maintaining your personal information. In order to protect your privacy, it is important to NOT save your Username and Password on your computer if you are prompted. If you save your Username and Password on your computer, anyone using your computer can access your information.
When you first enroll, you'll need to make sure the information we have is correct. After that, you can confirm that your information is accurate and up to date by reviewing it on our Web site or calling our Service Center. If demographic information is incorrect you need to contact the appropriate contact at your employer to correct the information. Benefit enrollment information, however, can be changed within the My Benefit ExpressTM system.
